Location:  The Chronic Fatigue & Immune Disorders Research and Treatment Center (Chronic Fatigue Center) is located at 8720 N. Kendall Avenue, (also known as SW 88th St) Miami, FL 33176. The office is conveniently located near the Kendall Drive exit of the Palmetto Expressway (826) and U. S. 1. (South Dixie Highway).  Our office is right next to Baptist Hospital.  There is plenty of parking on either side of the building.  Our office suite is on the first floor and closest to the north (street) side of the building.

Office Hours:  Appointment hours for the initial opening of the clinic are Monday and Friday 9 AM until 5 PM.  The Administrative office can be reached Monday through Friday, 9 AM until 5 PM. Calls will be returned within one business day.

Referrals from Primary Care Doctors:  Yes, you will need a written referral from a Primary Care doctor in order to be seen at the Chronic Fatigue Center.  You will need this referral even if it is not required by your insurance company.  We cannot act as your primary care physician.  We need a referral, in writing, as well as the name and phone number of the doctor for future contact regarding your treatment plan.

The clinic will begin with a two day a week schedule.  Appointments will be offered on Monday and Friday only, for the first few months and ultimately expanding to a full time schedule.

Fees/Insurance: The clinic will be a fee for service clinic.  We kindly ask that you pay the fee at the time of your appointment.  We accept cash, check,  or credit cards.  When you leave the clinic, we will provide you with a comprehensive bill which you can submit to your insurance provider.  The amount of your reimbursement will depend on your provider, your plan, your level of out-of-network coverage and any deductible required by your plan.  We will do our best to make the bill provided to you as “insurance friendly” as possible.  We appreciate your cooperation with this policy.  We will, however, need a copy of your insurance card in order to process blood work.  We will draw blood at the clinic as a convenience and the lab will bill your insurance directly.

Clinic fees are as follows:

New patient initial assessment - Initial assessments are $894 for the comprehensive office visit with a practitioner.  Lab work will be processed directly through your insurance with no upfront payment needed for the labs.  Additional diagnostic tests, should they be needed, will range from $200 to $500 depending on the test. 

Follow up patient appointment - Follow up appointments will be $175 to $450 for the office visit plus any additional diagnostic testing that is necessary.

Reminder:  While we will not accept insurance for your clinic visit, we will, however, ask for a copy of your insurance card for your blood work and laboratory testing.  As a convenience for you, we will draw blood at the clinic and deliver it to the UM lab.  The lab will bill your insurance company directly. If you do not have insurance, we will arrange payment for your lab work with you directly.

Diagnostics: Some diagnostic testing will be performed in the clinic itself, other tests will be referred out to other providers.  Diagnostic testing performed at the clinic will be included in the bill provided. Payment and/or insurance submission for testing done at other facilities will be the responsibility of the patient in conjunction with that facility.  

Medicare:  This clinic is not a Medicare provider and cannot bill Medicare at this time.   Medicare does not allow patients to self-submit.

Missed appointments:  Because your time is valuable as well as ours, we pledge to respect the time you spend at our clinic.  We will do everything we can to use your time efficiently and keep our appointments running on time, your help with this is greatly appreciated.  Because of the long appointment time (up to an hour with the Nurse Practitioner) There will be a $100 non-cancellation fee for any appointment not canceled within 24 hours of your scheduled visit. If you are too unwell to come to the clinic in person, we can arrange a video or phone consult.  However, please be aware that most insurance providers will not reimburse this cost.  Please contact the clinic for more information on this option.

Disability paperwork:  We are more than happy to fill out your paperwork for disability claims.  Disability paperwork will be completed on a fee basis depending on complexity of forms, number of pages and time required to complete.
